This is a reporting form for businesses that ship or market solid waste, recyclables and/or compostable materials to a facility not licensed by CSWD within or outside of Chittenden County.

Please complete this form by March 1 annually for the previous calendar year.

This form covers all materials that:
Are considered trash, recyclable, compostable, or animal feed
Were generated within Chittenden County
Were NOT marketed or shipped to a facility licensed by CSWD within or outside of Chittenden County
